How it works
A boss bar takes several commands: bossbar add creates it, and each bossbar set changes one setting. The generator only writes the settings that differ from the game’s defaults (white, smooth, 0 out of 100, visible), plus the players who should see it.

The name is written as SNBT instead of a JSON string. Everything else stays the same:

A new bar is invisible until you run bossbar set <id> players. To update it during a game, put execute store result bossbar arena:boss value run scoreboard players get #timer timer in a repeating command block or a ticking function. Remove it with bossbar remove arena:boss.
Bedrock and old Java
/bossbar is Java only and arrived in 1.13. Bedrock has no command for custom boss bars; maps there use an invisible boss mob with a custom name instead.
FAQ
Why can't I see my boss bar?
New bars have no players. Run bossbar set <id> players @a (the generator adds it when you fill in “Players”). Also check the bar is visible: bossbar set <id> visible true.
How do I make a boss bar count down?
Set max to the number of seconds and lower the value every second, or copy a score into it with execute store result bossbar <id> value run scoreboard players get <holder> <objective>.
What do the notched styles do?
notched_6, notched_10, notched_12 and notched_20 draw that many segments on the bar. They only change the look; value and max work the same.
Can I use /bossbar on Bedrock?
No, Bedrock has no /bossbar. Boss bars there only come from boss mobs such as the wither or ender dragon.
What is the bar ID?
A resource location like arena:boss or just timer (which means minecraft:timer). You use it in every later bossbar set, get or remove command.
